Medical Practitioner’s Signatures for Medication Administration (Schedules II-V)

The following memo was sent by the Tidewater EMS Council on 3/17/2015:

“Starting with shift change on the morning (after 0600) of Monday March 23, 2015, EMS providers in the Tidewater Region will be required to obtain a “Wet” Medical Practitioner’s (Physician, Physician Assistant, or Nurse Practitioner) signature on the pharmacy’s exchange form when exchanging Red (Drug) Boxes that have used Schedules II-V drugs (Morphine, Valium, Ativan and Versed).

Note: Practitioner’s Signatures will not be required for IVs, glucometer or other drugs (Schedule VI) not listed above, but is required Patient Care Report for field terminations [of resuscitation] made after Advance Life Support skills/procedures have been started/performed.

Note: The pharmacy at Childrens Hospital of the Kings Daughters (CHKD) will still require “wet” signatures for all medication (Schedules II-VI) administrated by EMS Providers.”
